I authorize Mai Vu Plumbing to charge the credit/debit card I've provided for the service fee and/or the total approved estimate/invoice amount. This charge will be processed within 24 hours of job completion.

If I'm not present when the work finishes, I authorize Mai Vu Plumbing and its representatives to close out the job on my behalf. I agree that a line placed in the signature block by the technician will serve as my acceptance of the completed work.

Mai Vu Plumbing is only responsible for installing the work explicitly detailed in your service agreement. I understand and agree that Mai Vu Plumbing is not responsible for the condition or functionality of existing parts (e.g., valves, pipes) that were used or are adjacent to the work performed but were not directly installed or replaced as part of this service.

Any additional plumbing work required due to unforeseen conditions of existing pipes or fittings will be a separate charge. This includes, but isn't limited to: slow drain lines, water or drain lines within walls or floors, compromised drains or traps, rotted galvanized fittings, broken closet flanges, leaking shut-off valves, or any other pre-existing conditions or plumbing needs beyond this estimate's scope.

Note: Existing plumbing in poor condition can develop issues after work is performed, even if those issues weren't present beforehand. Mai Vu Plumbing isn't responsible for these subsequent issues.

For areas where sealants are used, it's necessary to keep them dry for 24 hours after completion. Please do not wipe or clean these areas until they are fully cured.

An access panel may need to be cut for plumbing repairs and inspections. Unless specified in your service estimate, Mai Vu Plumbing is not responsible for any associated drywall repair.

For all work in apartment buildings and/or condominiums, you must provide a contact person and phone number for someone responsible for shutting off water to the unit.
